🎯 Take-home Message
  • Damage biomarker(TIMP-2*IGFBP7、NGAL、KIM-1、L-FABP)依其 regulatory approved 適應症使用以早期識別 moderate/severe AKI(成人 2B、兒童 2C)— PrevAKI 與 BigpAK 兩個 RCT 確立其指引價值。
  • Urine microscopy(muddy brown casts、RTE cells)在病因不明時做(2D);renal angina index(兒童 ICU)正式入指引(2B)。

KDIGO 2026 AKI/AKD — Figure 5
原文 Figure 5|Figure 5 | Conceptual framework for AKI risk stratification. This figure illustrates a framework for classifying people by AKI risk across a continuum of clinical severity, integrating susceptibilities (e.g., age, comorbidities) and exposures (e.g., nephrotoxins, surgery, sepsis) (Figure 4) with clinical scoring systems, risk models, or biomarker data. Using these tools, people can be stratified into low, moderate, or high risk of AKI or related outcome to inform management. This approach supports proactive personalized AKI prevention, early recognition, and individualized management. AI, artificial intelligence 2.1. Risk prediction models Recommendation 2.1.1: We suggest using externally validated clinical risk models to identify children and adults at increased risk of AKI and guide the use of kidney- protective strategies (2C). This recommendation emphasizes the use of externally validated clinical risk models for early identification of children and adults at increased risk of AKI. Such models integrate multiple patient-level and contextual risk factors and have demonstrated significant discrimination and calibration for predicting AKI, supporting more consistent and objective risk stratification. Applying these tools in susceptible populations may enable more appropriate and efficient care through timely implementation of kidney-protective strategies to people at higher risk. This approach can also avoid unnecessary testing and intervention in people who are unlikely to benefit. Overall, this approach supports proactive, evidence- informed personalized care aimed at reducing AKI incidence and associated morbidity.

Electronic AKI alerts Recommendation 2.2.1: We recommend against delivering interruptive, creatinine- based electronic alerts for AKI in isolation without systematic strategies for response in routine clinical care of adults (1B). This recommendation applies to interruptive electronic alerts triggered by changes in SCr and implemented without an accompanying, structured response pathway. Electronic alert systems require significant effort and information technology infrastructure to develop and maintain. Evidence indicates that alerts may increase AKI recognition and have modest effects on clinical processes of care but alone do not consistently improve patient outcomes and may contribute to alert fatigue. If electronic AKI alerts are used, they should be

Figure 6 是給 EHR 部署團隊看的 — 預測模型不是「導入就結案」,而是要 pre-implementation(development → validation)+ post-implementation(continuous monitoring → recalibration)的全生命週期管理。

SCr alert

多個大型 RCT(Wilson NEJM 2015、Selby JAMA 2019、Tachet du Borgne JAMA 2024)一致顯示 — 孤立的 SCr alert:

  • 但增加 alert fatigue(其他重要 alert 反而被忽略)

2.3 Kidney functional reserve、Furosemide stress test、KeGFR —

2.3.1 Kidney functional reserve(KFR)

Table 16:

  • Oral protein loading(最生理但變異大)
  • Amino acid infusion(standardized 但需 IV)
  • Creatinine clearance before / after protein load(需 24h 收集)

2.3.2 Furosemide Stress Test(FST)

KDIGO 2026 AKI/AKD — Figure 7
原文 Figure 7|Figure 7 | Application of the furosemide stress test in the setting of early mild-to- moderate AKI in adults.

Step 1:確認病人是 Stage 1–2 AKI,且 euvolemic 或 hypervolemic(不能在 hypovolemic 病人做 — 會誘發 hypotension)。
Step 2:排除 hypovolemia、obstruction、furosemide allergy。臨床上常見 pitfall:以為 hypervolemic(看到肢體 edema)但其實 intravascular 是 hypovolemic(septic shock 第三間隙)— 做之前用 POCUS 看 IVC 與心室填充。
Step 4a(無 loop diuretic 病史):給 furosemide 1.0 mg/kg IV。
Step 4b(有 loop diuretic 病史):給 furosemide 1.5 mg/kg IV(劑量加高補償 tolerance)。兒童版閾值不同(<2 ml/kg/2hr)。
  • ≥200 ml/2hr → likely reversible AKI(可逆性高,常規腎保護即可)

做之前先用 POCUS 評估 IVC、用 dynamic indices(PPV、SVV、passive leg raising)確認非 fluid-responsive。

2.3.3 Kinetic eGFR(KeGFR)

2.4 Biomarker —

[TIMP-2]*[IGFBP7](NephroCheck)UrineTubular stress(cell cycle arrest)PrevAKI(2017):cardiac surgery + biomarker >0.3 → KDIGO bundle 介入 → AKI 從 72%→55%。

KDIGO 2026 AKI/AKD — Figure 8
原文 Figure 8|Figure 8 | Associations of CKD staging by eGFRcr and UACR categories and risks for AKI in multivariable-adjusted analyses. Numbers reflect the adjusted hazard ratio compared with the reference cell. Adjustment variables included age, sex, smoking status (current, former, or never), systolic blood pressure, total cholesterol, high-density lipoprotein cholesterol, body mass index, use of antihypertensive medications, and a medical history of diabetes, coronary heart disease, stroke, heart failure, atrial fibrillation, peripheral artery disease, cancer, and chronic obstructive pulmonary disease, where relevant. The colors were determined for each outcome separately using the following rule: the percentile shaded the darkest green color corresponds to the proportion of cells in the grid without CKD (e.g., 6 of 35 cells with eGFR ≥60 ml/ min per 1.73 m2 and ACR. AKI, acute kidney injury; CKD, chronic kidney disease; eGFRcr, estimated glomerular filtration rate by creatinine; UACR, urine albumin-to-creatinine ratio

橫軸是 UACR 5 級,縱軸是 eGFR 7 級,網格內數字是 AKI hazard ratio(vs reference 即 eGFR 90–104 + UACR <10)。

  • 對心臟科:HF + albuminuria(即使 SCr 正常)= AKI 高風險,cath/contrast 前要特別小心

2.5 Etiology 與 Diagnostic Evaluation

2.5.1 Urine microscopy(尿沉渣鏡檢)

(Practice Points 2.5.1–6):

AIN vs other AKI
  • Cirrhosis-associated AKI 鑑別:FeNa <0.1% + plasma/urine NGAL 可協助區分 HRS(low FeNa, low NGAL)vs ATI(low FeNa 但 NGAL 高)
Urine sediment finding對應 syndrome
Muddy brown granular casts、RTE cellsAcute tubular injury (ATI)
RBC casts + dysmorphic RBCGlomerulonephritis
WBC casts、eosinophils(有限)AIN(acute interstitial nephritis)
CrystalluriaDrug-induced crystalline nephropathy(acyclovir、sulfa、indinavir、methotrexate)
Hyaline casts onlyPrerenal(hemodynamic)—
Pigmented casts + heme dipstick(+) without RBCRhabdomyolysis / hemolysis

2.5.2 Kidney biopsy

Renal Angina Index

(Table 20):

Risk score(×)Injury score
1 = ICU 入住 (low risk)
3 = solid organ / BMT
5 = mechanical ventilation + vasoactive
1 = SCr 上升 < 0.5× baseline(相對 baseline);UO ≥ 1 ml/kg/h
2 = 上升 0.5–1.5×;UO < 1 ml/kg/h
4 = 上升 ≥ 2×;UO < 0.5 ml/kg/h
8 = 上升 ≥ 3×;UO < 0.3 ml/kg/h

RAI = Risk × Injury;≥8 為「renal angina」陽性 → 高 severe AKI 風險,需要積極腎保護與 biomarker 配合。

  • HF 病人加重時的 SCr 上升:先做 POCUS(IVC、B-lines、心室)區分真正 volume-overloaded(要 diuresis)vs effectively hypovolemic(要 fluid)。

Recommendations

#Recommendation / Practice PointGrade
2.2.1反對單純 SCr-based interruptive EHR alert(無 systematic response strategy)1B
